Physician Coat Ordering Instructions

This form should be used for ordering physician and care provider white coats. Please be sure to follow all instructions carefully. Click here for the price list.

Each coat will be embroidered with two lines of information: the provider’s department and name. Exceptions must be submitted to the Associate Dean’s office for approval.

Sizes are available for both men's and women's coats (women's sizes go up to size 52). Also below is a sample of how the embroidery will appear (on the right front chest). Not sure what size to order? Samples of all sizes are available to try on in the Lab Coat Room, B2G403A. The room is open on Mondays and Fridays from 8 a.m. to 12 p.m., and on Tuesdays and Thursdays from 12 p.m. to 4 p.m.
